This policy explains how FORMe Health uses the information you share with us through this website and when you contact us.
Who we are
FORME Health Ltd, trading as FORMe Health, is the data controller for your information. We are registered with the Information Commissioner's Office (registration number ZC152544). Our address is 25 White Hart Lane, Barnes, London SW13 0PX, and you can contact us at info@forme-health.com.
What we collect
When you register your interest, send an enquiry or request an appointment, we collect the details you enter: your name, email address, telephone number (if you give one), the services you are interested in, your contact and appointment preferences, anything you choose to write in a message field, and whether you have opted in to news and updates. We also record when your enquiry was received.
Our forms ask only for brief details. Please share only what is needed for us to respond. We will gather anything clinically relevant with you directly and securely as part of your care.
Why we use it and our lawful bases
Where you ask us to arrange or provide a service, we use your information to take steps at your request before providing that service and to provide it (UK GDPR Article 6(1)(b)). For other enquiry administration, such as replying to general questions and keeping a record of our correspondence, we rely on our legitimate interests in responding to people who contact us (Article 6(1)(f)).
Where we use health information to provide healthcare, we rely on Article 9(2)(h) of the UK GDPR, together with the relevant provisions of the Data Protection Act 2018. This information is handled by health professionals, or people acting under a duty of confidentiality, in line with professional confidentiality obligations.
We only send news and updates if you have chosen to receive them (Article 6(1)(a)).
Who can see your information
Website enquiries are stored in a private database that is not publicly accessible. Email notifications about new enquiries are sent only to Leslie Abrahams's FORMe Health inbox. Clinical records are kept separately in Nookal, our practice-management system. We do not sell your information or share it for advertising.
Service providers
We use trusted service providers to run this website and our practice: Lovable (website and enquiry storage), Cloudflare (website delivery and security), Google Workspace (email) and Nookal (clinical records and appointments). They process information on our behalf and only for the purposes we set.
How long we keep it
- Enquiries from people who do not become clients: 12 months after our last contact.
- Enquiries that become part of a clinical record: relevant details are transferred to Nookal, and we aim to remove duplicate website and email copies within 30 days, subject to any retention requirements.
- Pre-launch registrations without marketing permission: six months after the clinic opens.
- Marketing records: for as long as we have a valid purpose and your permission, reviewed at least every two years.
- Clinical records are held separately in Nookal, our clinical records system.
Children and young people
We accept enquiries about children and young people. A parent or carer may contact us on a young person's behalf. Consent, confidentiality and parental involvement are considered according to the young person's age, understanding and clinical circumstances, and our safeguarding responsibilities. A parent or carer does not automatically have unrestricted access to a young person's clinical information.
Marketing consent
The marketing consent box is never pre-selected. You can withdraw consent at any time by contacting us, and every marketing email will include a way to unsubscribe. Withdrawing consent does not affect our reply to an enquiry you have already sent.
Your rights
You have the right to ask for a copy of your information, to have it corrected or deleted, to restrict or object to how we use it, and to withdraw consent where we rely on it. These rights may be limited in some circumstances, for example where we must keep clinical records. Contact us using the details above and we will respond within one month.
If you are unhappy with how we have handled your information, please contact us first. You can also complain to the Information Commissioner's Office (ico.org.uk).
