Why we collect it

We use your details to respond to your enquiry, to let you know when appointments and sessions become available, and to arrange care with you. Our lawful basis for this is our legitimate interest in responding to people who contact us, and taking steps at your request before entering into a service with you.

How enquiries are handled

Enquiries are stored in a private database that is not publicly accessible and is only reachable by this website's server. They are seen only by the FORMe Health team. We do not sell your information, and we do not share it with third parties for advertising. We keep enquiries only for as long as we need them to respond to you and maintain our records, and then delete them.

Marketing consent

We only send news and updates if you have ticked the marketing consent box, which is never pre-selected. You can withdraw that consent at any time by contacting us, and every marketing email will include a way to unsubscribe. Withdrawing consent does not affect our ability to reply to an enquiry you have already sent.

Your rights

Under UK GDPR you have the right to ask for a copy of the information we hold about you, to have it corrected or deleted, to restrict or object to how we use it, and to withdraw consent where we rely on it. To make a request, contact us using the details below and we will respond within one month.
